We recognize that gymnastics is a sport that purposes in any medium of advertising, communication, publication or publicity that will promote G.A.B. programs, involves inflatables, height, and rotation of the body and there are inherent risks involved. On behalf of our child and on our own behalf, we agree to waive all claims against Gymnastic Academy of Boston, Inc and its owners, staff and instructors for any liability, loss, cost, damage, medical expense, long-term care or emotional distress arising out of any personal injury, including total disability, paralysis and death, which may occur to any of our children while on the premises of or under the instruction, supervision, or control of Gymnastic Academy of Boston, Inc. We hereby testify to our child’s sound health of mind and body and we authorize Gymnastic Academy of Boston, Inc to seek medical treatment at the nearest medical facility in the event of an emergency. I have *read and understand all terms, including the Risk and Waiver of Liability as posted on this form. Make-up Classes- Make-ups are provided as a courtesy for actively enrolled students, as space permits and are limited to one per month. Once a makeup is scheduled it cannot be rescheduled for another date, as we are reserving the space for your child. Make-ups need to be scheduled in advance by phone, email or in person. Missed classes cannot be deducted, credited nor transferred. Discounts- The following discounts will automatically be calculated: 20% sibling discount, 20% off each additional class for one student. Discounts are applied to the less expensive class and are not to be combined with sibling discount. Membership also qualifies members for additional discounts for parties, camps, special events and activities. Holidays- We will be closed on Thanksgiving, Christmas, New Year’s Day, Easter and Independence Day. No classes will be held during the December and February school vacation weeks. Camps- Please inquire about our FLIP, FLOP and TUMBLE vacation programs and summer camp. Spring Show- Our annual spring show is always an exciting time. THE KIDS LOVE IT!! All classes for ages 3 and up will participate. The first week in June will be our end of the year celebration to show off the wonderful skills they learned all year with the friends they have made. After practicing their skills and routines for months they will get the opportunity to show off! As part of the day, children wear a special-ordered theme oriented leotard or T-shirt to coordinate with the theme of the show! All show ttire must be paid for by Quarter 3. Family and friends are invited! Drop Policy – Gymnastics academy of Boston is a school year program. If you wish to end enrollment, written notice or a drop form is required a minimum of 2 weeks prior to the upcoming quarter. Gymnastics Academy of Boston does not issue refunds, credits, pro-rate or guarantee make-ups for missed classes due to personal reasons and you are obligated to pay the full tuition once a session begins.
